Player Movement and Process Form -  With the implementation of the two new forms within the AA Hockey Model, Hockey Alberta’s “Minor Hockey Player Movement Form” has been amended. Amendments to the “Minor Hockey Player Movement Form” include the removal of the option “I would like to try-out for a AA team” and the rewording of the option “My Resident Association and this Association have joined together to form a team”. Additionally, information on carryover suspensions has been added

Overage Criteria Application - player must remain in division registered according to their birthdate - player can not be moved to lower division until application has been approved. 

Parent-Guardian Declaration - for players who have moved into Peace River Minor Hockey's boundaries from another association.  Declaration form must be submitted with registration documents.

Player Medical Information Form - for Coaches/Managers to complete once team has been selected.

Request for Registration Refund - to be completed when player has withdrawn from hockey
