 2024 PRMHA Sanction and Travel Permit Request Form - to receive a travel permit you need to email The Director of Directors at Trevor Massier with as much information as possible. The turn around can be up to 1 week so please submit as soon as you need one.

Travel permits and sanction numbers are not required for the league tiering games or regular league. 

Please remember that this season Hockey Alberta has made it mandatory to have a travel permit for every away game (excluding league) even within the zone. Also, blanket permits are not allowed anymore so every U7 and U9 game requires an exhibition game permit because the league does not schedule them.

Electronic Game Sheets - Need to be submitted electronically with Hockey Alberta, U7 and Up for any exhibition game or tournament. (Home Team)

Hockey Alberta Injury Report Forms - keep these on hand. Any injury a player has please fill one out for your records and there safety in case anything comes of the injury in the future.

Hockey Canada Return to Play Form - must be filled out by a doctor and return to your team manager, prior to returning to play

Respect In Sport Activity Leader - mandatory for anyone on the ice or the bench with the players. Including managers.

ALL Team Officials are required to submit a recent RCMP Vulnerable Sector Check and recent Intervention Records Check (Children Services Office). This needs to be submitted to the peaceriverminorhockey@gmail.com

All Peace League Quick Links

All Peace Ramp Instructions - each manger and coach was emailed the username and passwords to your team for the all peace league. Look over these steps to log into your team. Enter Rosters, Add affiliates, enter game sheet info.

APHL Game Change Form - game change form is very important. you and the other team must most fill out the attached form and submit it to your commissioner for approval. When doing a game change, for a home game please make sure your double check ice with Administrator, Erin Bandurka at 780-617-3431 and Referees with Troy Kish at 780-219-0370. 

APHL Notice of Suspension - notice of suspension form. There should be an updated form. This is what you will receive from your commissioner in regards to a player on your teams suspension results. It will be sent to the Administrator and President of PRMHA.

APHL Suspension Served Form - Once the player/coach suspension is served. Please submit a copy to your Commissioner via email.
